We are aware that there is a problem with our online forms. If you try to enter a single digit day (for example, 8/1/2026), the form is rejecting whether you enter 8/1/2026 or 08/01/2026 or you use the calendar picker.

Our form designer will be looking into this problem by Monday and hopefully this will be an easy fix.

UNTIL THE FORMS ARE FIXED, WE ARE RECOMMENDING THAT PROVIDERS HOLD OFF TRYING TO SUBMIT ANY FORMS.

DO NOT SUBMIT A FORM, LIST A WRONG DATE AND TRY TO INDICATE IN THE NOTES THE CORRECT DATE; FORMS LIKE THIS WILL BE DISCARDED.

We will post notice here in the Support Suite once the problem has been fixed.
